Conifer, Welldoc partner in digital health app

Conifer Health Solutions, a revenue cycle and value-based care solution provider, has teamed up with Welldoc to launch a new digital health app, Conifer Connect.

The new app, offered through the Conifer Health Solutions Personal Health Management program, aims to help members manage their daily health. The app connects to Welldoc’s platform to provide individualized insights.

Members of the app will have collaborative care experience through data aggregation, risk stratification and care coordination.

Together, we're thrilled to launch Conifer Connect, helping our members to engage with their personal health nurse and self-manage their care to strengthen clinical interventions."

The Conifer Connect App syncs, through Welldoc platform’s advanced AI-engine, with more than 400 smart devices, such as fitness trackers and blood glucose meters, to track vitals, medications, food, weight, activity, blood pressure and sleep. Members of the app receive daily and weekly trends to inform health decisions and keep their personal health nurse (PHN) informed.

“Partnering with Conifer Health Solutions will provide members of their Personal Health Management program with an enhanced member experience by connecting their own data to power personalized, AI-driven digital coaching and insights, further supporting self-management and improved outcomes,” Keith Reynolds, chief operating officer of Welldoc, said in a statement. “Conifer Connect is taking coordinated care to the next level, strengthening members’ connections with their personal health nurse to help them lead a healthier life with ease.”

The app launch comes as more healthcare providers are leaning on digital apps and artificial intelligence (AI) tools to manage health data, but patient monitoring systems are also becoming more and more common. Conifer Health Solutions is a subsidiary of Tenet Healthcare, manages more than 17 million unique patient interactions, more than $25 billion in net patient revenue and more than $22.6 billion in medically managed spend.
