N.J. provider expands post-discharge care management program

Pharos Innovations has partnered with South Jersey Healthcare (SJH), a regional healthcare provider in southern New Jersey that serves a rural population, to expand SJH's post-discharge care management program to include the monitoring and management of patients with chronic obstructive pulmonary disease (COPD).

Through the current heart failure program, SJH reduced hospital admissions for enrolled patients, and the expansion will help improve access to care and care quality for patients with heart failure, according to Northfield, Ill.-based Pharos, an interactive voice response and web-based remote patient monitoring technology provider.
