AHRQ offers toolkit for medication reconciliation programs

The Agency for Healthcare Research and Quality (AHRQ) has made available the medications at transitions and clinical hand-offs toolkit to assist providers developing or revising medication reconciliation programs.

The toolkit promotes compliance with the Joint Commission’s national patient safety goal for communicating accurate patient medication information, provides a framework for using EHRs to completely capture accurate medication information and offers plans to redesign existing medication reconciliation programs or to create one from scratch.

Available work plans offer suggestions on achieving support of leadership, building project teams, designing a medication reconciliation process, implementing a medication reconciliation process, program training and process evaluation.
