AdventHealth partners with Monogram Health for remote monitoring program

Health system AdventHealth has teamed up with Monogram Health to improve the health outcomes of kidney care patients. 

Monogram Health is a value-based specialty provider of in-home nephrology, primary care and benefit management services for individuals with chronic kidney and end-stage renal disease. AdventHealth is a non-profit health system based in Florida with operations in nine states. The partnership brings together Monogram’s specialized MSO kidney care teams with AdventHealth providers with the intent to slow the progression of chronic kidney and end-stage renal disease.

“Monogram Health’s broad range of kidney-focused services will serve as a valuable resource to AdventHealth providers in the shared pursuit of improved clinical outcomes,” Thomas Diller, MD, chief medical officer of AdventHealth’s Population Health Services Organization, said in a statement. “Monogram’s evidence-based, in-home care management model offers a blueprint for providers as they work to delay the progression of kidney disease, prevent complications and appropriately prepare and manage patient needs.”

Monogram Health serves patients across AdventHealth’s Florida markets, including members enrolled in AdventHealth Advantage Plans MA, AdventHealth Employee Plan, AdventHealth Advantage Commercial Plans and AdventHealth Well 65+. Monogram is working with AdventHealth to coordinate care for patients, including with nephrology specialists and primary care providers, dialysis, transplant, palliative care and social services; family counseling; in-home care management; medication management; advance care planning; nutrition management; referral management; remote monitoring/biometrics; and telehealth. Monogram’s services are available to patients at no cost. 

The partnership comes as more healthcare organizations are focused on managing chronic conditions of their patients, as these conditions tend to be the most costly. Early interventions and coordinated care can help improve care gaps and keep patients from rising to higher acuity levels.

The kidney care market recently saw major consolidation earlier this year, when InterWell Health completed its merger with Fresenius Health Partners and Cricket Health in a deal valued at $2.4 billion.
