Electronic health records (EHR), or electronic medical records (EMR), are used to records all of a patient's data and imaging in one location to make it easier for clinicians to access.

Researchers with UTHealth Houston outlined five steps for clinicians to assess their electronic health records (EHR) using the CMS SAFER guidelines.

 "This research aims to raise a red flag regarding naive off-label usage of open-access data in the development of machine-learning algorithms," experts involved in the study said.

These quinapril HCl/hydrochlorothiazide tablets, which Pfizer sells under the name Accuretic, are included in the voluntary FDA recall.

The voluntary recall includes quinapril HCl/hydrochlorothiazide tablets sold by Pfizer and authorized generics of that same medication.
