R.I. payor, physician group launch patient-centered home care

Blue Cross & Blue Shield of Rhode Island (BCBSRI) has entered into an agreement with the Rhode Island Primary Care Physicians Corporation (RIPCPC) to establish the largest patient-centered medical home (PCMH) program in R.I.

With 162 physicians and approximately 300,000 patients—about 120,000 of whom are BCBSRI members—RIPCPC is the largest provider of primary care in the state. 

Emphasizing preventive, proactive and coordinated care, PCMH models are designed to improve patients' health and moderate long-term healthcare costs. BCBSRI members, especially those with complex conditions, such as cancer, heart disease or chronic renal disease, will benefit from this care, BCBSRI said.

Every aspect of a patient's care, including treatment options, drug regimens and appointment scheduling, will be coordinated through the primary care physician, with support from a care team that will work together to improve that patient's health. To facilitate the adoption of its PCMH model, BCBSRI will provide funding to support RIPCPC efforts to implement EHRs in each of its practices, the payor said.
