N.Y. payor invests $1M for medical home initiative
CDPHP, a physician-founded health plan based in Albany, N.Y., will invest $1 million to compensate 21 primary care and pediatric practices for participating in the second phase of the healthcare plan’s patient-centered medical home (PCMH) initiative.
Since 2008, CDPHP said it has been working with area physicians to adopt a medical home model for primary care. This month, the payor and 21 additional practices commenced the second phase of a medical home initiative capable of reaching nearly 52,000 members and encompassing more than 160 network physicians.
Practice selection for both phases was based on demonstration of leadership at the practice level, stable practice culture, volume of served CDPHP members, use of IT (EMR/e-prescribing) and a willingness by practice leadership to participate and collaborate with the payor.
The investment will help Phase II participants achieve PCMH level III certification by providing facilitated support via TransforMED Consulting, integration of CDPHP resources (case management, pharmacy, behavioral health and data analytics) where appropriate, and stipends to cover start-up costs.
Practices that earn Level III certification and have been active participants in the CDPHP-sponsored transformation process will be eligible to participate in an innovative payment model commencing in the fall of 2011. The move to the new payment model will be contingent on the outcomes of the pilot evaluation, according to CDPHP.
“The ultimate vision for this program is to create an innovative and sustainable model for the reimbursement of primary care physicians, leading to a resurgence of interest in primary care medicine as a career," said Bruce Nash, MD, MBA, CDPHP senior vice president and chief medical officer, who has overseen this initiative since its inception.
