MedEx, Rite Aid partner to lower hospital readmission rates by 15%

Readmission rates among patients can rise when patient aren’t well informed about their care. In an effort to minimize readmissions, Patient Portal Technologies partnered with Rite Aid pharmacy to significantly readmission rates by implementing its MedEx program in Crouse Hospital.

"We implemented bedside delivery for discharge medications as a benefit to our patients. In less than a year we saw a 15 percent reduction in readmissions in the study group. Those that received delivery have found this to be an incredibly convenient and beneficial service, based on patient satisfaction surveys" stated Barbara Drapola, Director of Care Coordination Services at Crouse Hospital. 

The MedEx program offered free services to hospital patients access to a pharmacist before and after care for counseling and review, and a medical concierge portal that helped patients navigate through their care. With all of these services there is peace of mind for the patient who is able to understand his own healthcare, to become more proactive and reduce readmission rates. Patients using MedEx reported satisfaction of 98 percent, 97 percent found the program helpful and 95 percent would recommend it.
