Health systems target high-cost Medicaid patients

Health systems are increasingly focusing on caring for patients with Medicaid who have costly medical conditions, The New York Times reports.

A large number of these patients have a combination of poverty, homelessness, mental illness, addiction and past trauma. There are numerous experiments run by states, private insurers and healthcare providers examining care for these patients.
