Eccovia Solutions brings care coordination to Los Angeles health services

The Los Angeles County Department of Health Services has selected Eccovia Solutions, a provider of healthcare coordination platforms, to develop a care coordination platform for the county’s 1115 Waiver Whole-Person Care (WPC) pilot program.

The ClientTrack Care Coordination platform will be used to bring primary care and community resources together, aiming to improve the sharing of patient information. The platform allows for collaboration between many different entities as well as data visualization to measure effectiveness.

The WPC program plans on developing a health system focusing on 50,000 of the most in-need people in the largest county in America. The program plans on expanding healthcare at lower costs with 11 programs to address an array of needs:

The 11 programs include:

  • Homeless care support services
  • Tenancy support services
  • Recuperative care
  • Sobering center
  • Benefits advocacy
  • Re-entry enhanced care coordination
  • Community-based re-entry
  • Intensive service recipients
  • Residential and bridging care
  • Substance abuse disorder engagement, navigation and support
  • Transitions of care

“This care coordination platform will provide the technology infrastructure for Whole Person Care-LA and enable our teams to connect the most vulnerable Los Angeles county residents to services,” said Clemens Hong, MD, MPH, director of Whole Person Care Los Angeles. “This will improve the overall well-being of these individuals and decrease the use of high-cost services.”
