Allscripts unveils population health analytics tool

Allscripts announced the launch of Population Health Analytics, a population health management tool that builds analytics into a clinician’s workflow to improve chronic disease management and reduce readmissions, according to an Aug. 19 announcement.

The solution will provide real–time performance indicators within an EHR to help identify target populations and guide clinical intervention for chronic conditions, such as diabetes, heart disease, coronary artery disease and asthma. The tool aims to improve care quality and create better outcomes for patients and the overall population.

"The introduction of Population Health Analytics marks a shift from reactive to proactive care. This solution reflects the move from individual patient care to whole population health, an essential change as we make the move towards value-based care," said Paul M. Black, president and CEO of Allscripts.
