The Office of the National Coordinator for Health IT has posted a video for providers and professionals on five practice transformation initiatives: medical homes, accountable care organizations, the comprehensive primary care initiative, the state innovation models initiative and Million Hearts. The video provides an overview of each initiative, how providers can benefit from being involved in each initiative and how regional extension centers can help providers with each initiative.
