e-MDs, Lightbeam Health and Princeton HealthCare System Partner to Enhance Population Health

AUSTIN, TexasFeb. 10, 2015-- e-MDs, a leading provider of electronic health records (EHRs) and practice management (PM) solutions, and Lightbeam Health Solutions, LLC, a leader in end-to-end population health management, announced today the selection of their integrated platform by Princeton HealthCare System. The integrated solution will enable Princeton HealthCare System's outpatient practices to replace several other disparate systems with a single, powerful tool. 

Princeton HealthCare System will leverage the system to better identify the needs of its patients based on select, detailed criteria – especially those patients at higher risk who require advanced levels of chronic care management. The system will also enable providers to access and track when a spectrum of health parameters and quality metrics have been achieved for a defined patient population.
