Humana and HealthCare Partners Nevada Launch Accountable Care Agreement in Nevada

 

LAS VEGAS Humana Inc. (NYSE: HUM) and HealthCare Partners Nevada announced a new Accountable Care arrangement and population-health partnership in support of their continued efforts to provide improved health and coordinated care for Medicare beneficiaries and commercial members.

The agreement expands on Humana and HealthCare Partners Nevada’s longstanding relationship. As part of the multi-year partnership, HealthCare Partners Nevada has opened three new primary care practices in Las Vegas this year under HealthCare Partners Medical Group.

The new clinics provide comprehensive primary care services for Humana Medicare Advantage (MA) members, original Medicare beneficiaries and patients with most commercial insurance plans. All HealthCare Partners locations practice the organization’s Total Care Model, a unique approach to health care in which primary care providers, specialists and professional staff work as a coordinated team to manage all aspects of patients’ care and overall health. This includes a strong focus on prevention and assisting patients with maximizing their health insurance benefits.

The new primary care practices are located at:

  • Boulder Crossing: 5230 Boulder Highway, Suite 110 in Las Vegas
  • Fort Apache: 4730 S. Fort Apache Road, Suite 150 in Las Vegas
  • Sun City: 9454 Del Webb Blvd. in Las Vegas           

The Fort Apache and Boulder Crossing locations began seeing patients in October 2014. The Sun City location began seeing patients last month.

With the three new clinics, HealthCare Partners Nevada now offers a network of 310 primary care providers at 98 primary care locations throughout Southern Nevada. The company also provides patients with access to more than 1,700 specialists.

The Humana and HealthCare Partners Nevada partnership will offer more personalized care and a broad range of population management tools like predictive analytics, chronic disease management and wellness programs.

The Humana/HealthCare Partners Nevada partnership emphasizes quality outcomes with a focus on care coordination and leverages measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS), such as diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication.

Humana has a 26-year accountable care relationship history with more than 1.2 million MA members that are cared for by 38,000 primary care physicians, in more than 900 accountable care relationships across 43 states and Puerto Rico.
