AHRQ guide aims to help hospitals tackle Medicaid readmissions

The Agency for Healthcare Research and Quality has released a guide to help hospitals pursue evidence-based strategies to reduce readmissions among the adult Medicaid population.

The guide specifically is designed to:

  • Help acute care facilities adapt or expand existing Medicaid readmission reduction efforts;
  • Develop Medicaid readmission reduction strategies using the guide’s roadmap featuring 13 customizable online tools;
  • Comply with the Centers for Medicare & Medicaid Services’ conditions of participation requirements for standard, improved and transitional care for all patients; and
  • Develop partnerships across other settings

“Hospitals at different stages of readmission reduction efforts can benefit from implementing this guide,” according to the agency.
