PointClickCare Strengthens Interoperability in Senior Care with Industry’s First Fully Integrated Continuity of Care Document

MISSISSAUGA, Ontario--Continuing in its efforts to enhance the communications between senior care providers, hospitals, and accountable care organizations (ACOs), PointClickCare, the leading provider of cloud-base software for senior care, today announced new capabilities to integrate continuity of care documents (CCD) and lab results into its EHR platform. This level of integration enables senior care providers to bolster interoperability with their partners and improve connections with their provider networks. These capabilities further PointClickCare’s efforts to enhance the communication between senior care providers, hospitals and accountable care organizations (ACOs), driving better care.

Collecting the required documentation to properly communicate resident needs at the time of transfer is time consuming, error prone and often incomplete. This can leave the hospital without sufficient, accurate information to provide the right care and services. Offering the senior care industry's first fully EHR-integrated CCD, PointClickCare’s platform automates the next generation of health information delivery. The receiving facility receives consistent data, in a uniform format, improving communication of treatment and care needs during transition of care.

"Using PointClickCare’s CCD, my organization is seeing success in exchanging quick and accurate resident data with hospitals,” said Coral Lindahl, RN-BC, CLNC, C-NE, RAC-CT, CCN-C, CPRA, PointClickCare Coordinator at Ebenezer Ridges Campus in Burnsville, Minnesota. “As a result, we are realizing more effective continuity of care between residents and practitioners; as well, we are strengthening continuity of relationships between long-term care and acute care. Overall, this translates to practitioners making better medical decisions and improved resident outcomes. PointClickCare is setting the standard in connected care. We would all benefit from this model.”

With PointClickCare's Integrated Lab Results™, senior care providers are now able to communicate directly and rapidly with ancillary diagnostic services and exchange resident information through the facility's EHR. When results begin to trend outside of normal ranges, they are automatically flagged for review, allowing the clinical staff to adjust care plans and deliver appropriate care.

“When it comes to accessing lab results for patients, time and accuracy are critical success factors,” said Meyer Tollen, CIO of TridentUSA Health Services. “Partnering with PointClickCare on Integrated Lab Results allows us to send electronic lab reports directly and securely to senior care staff via the PointClickCare EHR and create greater efficiencies in their workflow. By spending less time searching for results, the staff can devote more time to the patients for a better care experience.”

Providers using PointClickCare are able to search for participating labs that offer integration with the platform by visiting the PointClickCare Lab Exchange website.

"Great senior care depends upon integrating data with effective and consistent communication to those in the immediate network of a facility's care continuum," said Mike Wessinger, PointClickCare CEO. "Technology not only expedites this communication, but in many cases dramatically reduces errors and delays that are prevalent in paper-based transactions. The suite of interoperability capabilities from PointClickCare enables providers to become better integrated, better connected, and more streamlined with their partners in care."
