Three healthcare systems join CMS medical home pilot

The Centers for Medicare & Medicaid Services (CMS) has announced that three healthcare systems are joining 16 independent practices in the Independence at Home Demonstration, an initiative to provide home-based primary care to chronically ill patients with the goal of reducing hospitalizations and healthcare costs.

To participate in the demonstration, healthcare organizations must be physician led, have a history of providing home-based care and serve at least 200 eligible beneficiaries. The recently added healthcare systems are:
  • Innovative Primary Senior Care in Skokie, Ill.;
  • Treasure Coast Healthcare in Stuart, Fla.; and 
  • Virginia Commonwealth University Health System and the Medical College of Virginia Hospitals and Physicians in Richmond, Va.
