Telemonitoring lowers readmission rates at Geisinger

Patients enrolled in a telemonitoring-enabled case management program experienced a 44 percent reduction in 30-day hospital readmission rates compared to a control group that did not benefit from the technology, according to the results of a Geisinger Health Plan study of its two-year-old patient monitoring program.

Interactive voice response technology from AMC Health, a New York City-based health IT vendor, is a core component of the post-discharge comprehensive case management model employed by the Geisinger Health System, an healthcare organization headquartered in Danville, Pa. 

Geisinger providers can monitor approximately 1,000 heart patients remotely at any given time and patients have demonstrated an 85 percent compliance rate with the program, the provider added.
