Mirth population health platform to assist ACOs

Open source health IT service provider Mirth, through a license agreement with Pfizer Health Solutions, is offering InformaCare technology platform to clients to implement population health management initiatives.

InformaCare can serve as the technology foundation for community-wide care management initiatives such as accountable care organizations and patient-centered medical homes.

Mirth will make the platform available to its healthcare clients under the Mirth Care brand, the Irvine, Calif.-based open-source health IT service provider stated.

The Mirth Care platform integrates clinical records, claims history and patient-collected data to inform phone-based patient counseling, and supports telephonic outreach to engage patients, the company stated.
